Each provider in the Bluegrass Region receives 1 free copy of the CCC calendar.  Extra copies can be ordered online.  Click here to order additional copies.                                                      

Priority is given to participants registering online.  Mailed registrations will only be accepted if payment is attached to registration forms.  EFFECTIVE IMMEDIATELY, faxed registrations are only accepted if you are approved for purchase orders (PO's) with the CCC.
